Make a referral

Easter Respite has two referral pathways – one for respite services and the other for day programs. Below is an overview of each.

Respite Services

Respite services are for people with a disability aged 0 – 65. Respite services generally include Individual Respite, Out of School Care Programs (for school aged children) and weekend groups.

Eligibility

To be eligible to receive a respite service, the person with a disability must meet the following criteria:

have a physical, intellectual or sensory disability
be aged 0 – 65 years
live at home with their primary carer
live in one of the 5 local government areas in Eastern Sydney – Botany, Randwick, Sydney City, Waverley, Woollahra

Referrals

To make a referral you may either:

Contact Eastern Respite & Recreation’s Intake Officer on (02) 8347 7008. The intake officer will speak to you over the phone to ensure you meet the entry criteria and then post you a printed copy of the referral form to complete.

Please note: It is important to provide as much detail on the new referral form as possible as this will help us ensure we match the right services to you. If you would like help completing the referral form the intake officer is able to help you over the phone.

Once we have received your completed Referral Form it will be prioritised and placed on our waiting list. You will also be sent a letter acknowledging the receipt of your referral.

As vacancies arise, priority of service will be given to those carers/people in greatest need.

All referrals remain on our waiting list for a period of two years. After this period you will be contacted to update your initial referral. If you no longer wish to be on the waiting list or you are un-contactable at this time your name will be removed from our database.
